H-FLOW is a community-based platform. It was designed as a healthcare algorithm that interconnects a population-based epidemiological registry with a patient-centric clinical problem-solving methodology. The algorithm was developed respecting people’s needs and rights, promoting prophylactic community health actions and supporting a more inclusive and equitable healthcare system. Focused on the base of the health pyramid, it controls patient’s entry into the public and private healthcare system. The algorithm feeds the registration data into each personalized-DHR and initiates an ethically-driven clinical data flow among all healthcare stakeholders. A preliminary satisfaction assessment appears to meet the expectations of all parties. This was predictable considering that they all work for the benefit of people, the same people they depend on to achieve their goals.
