The instructions provided reinforce patient’s proactive behavior. The previous virtual contact allowed the doctor to understand the nature and extent of patient’s problems. Now, in the face-to-face consultation, the doctor already knows which specialties should be included to meet specific needs. During the face-to-face consultation, the patient will have all relevant information for shared decision making. Sequentially, the patient will have all clinical data stored in his/her DHR and the MRE medical will give him/her the evidence to support all recommendations. All clinical data will be immediatly available online. Using personal login and password, the patient will access his/her clinical data in real time, worldwide, through a computer, tablet or smartphone. Currently available technology would make it translated in anynatural language.
Here, you review the authorization to allow access, viewing and editing of your personal and clinical data, as recommended by the General Data Protection Regulation (GDPR). Here, you can check the authorization time, date and IP Device ID.
Here, all your personal data are stored, including permanent clinical information such as ABO and Rh group systems, allergies, immunizations and long-term care medications. In the registry, you will also find your attending physician name and licence number
Here, the attending physician will create a list of problems as described by Lawrence Weed. Before introducing a new problem, the doctor will review the entire list, avoiding repetition. Each problem listed is automatically followed by the date it was added
Here, the attending physician points out the clinical and laboratory findings that support each problem listed. These are preliminary data obtained in the anamnesis, physical examination, lab and imaging tests brought by the patient.
The information provided in the database generates diagnostic hypotheses and recommendations. In this icon, the attending physician requests additional tests to increase diagnostic accuracy and proposes a treatment plan. The attending physician also appoints consultant physicians, promoting comprehensive care
Here, your exams are stored. The exams are included by the attending physician and classified into categories. At your discretion, you may allow electronic access to reports, images and/or videos of your exams
Here, the attending physician enters progress notes. Each note is associated with a specific problem. In this icon, you will also find the reports of the consulting physicians, automatically dated and linked to their respective specialties
