The patient must be guided in complying with the recommendations and know his/her subsequent follow-up routine. Here, you can see how the guidelines provided to the patient lead your facility to achieve the third strategic goal.

Guidelines Provided to the patient

Here, you will receive a personalized care program. It consists of an integrative assessment involving all medical specialties necessary for your comprehensive healthcare. In general terms, the program covers the attending physician and all designated consulting physicians. The attending physician identifies the extent of your problem and guides the participation of consulting physicians, whose number varies according to your specific needs. Each consultant physician carries out his assessment and returns his recommendation to the attending physician through a medical conference. Clinical data are edited and published in your DHR and discussed on your customized tumor board, involving the attending physician and all your consulting physicians, resulting in consensual recommendations. Subsequently, the attending physician edits and publishes a medical report in your DHR, according evidence-based medicine.

Our dynamics also provides a personalized follow-up program, including all the specialties involved in your care. You will receive an appointment calendar. Your attending physician will complete it. It will indicate the scheduled date for each intervention and the date on which it is actually carried out. Remarks, when checked with an (Yes), must be fully described, pointing out the need for further investigation. Each follow-up intervention will be posted twice under the Progress Note icon of your DHR.

The guidelines provided in this section extends proactive patient behavior into his/her care and follow-up programs. The platform induces a cognitive and empathetic approach by the attending physician. It is inclusive in nature, allowing the patient to understand the reasoning used, promoting easy access to his/her clinical data. Additionally, Onco Qaly Platform makes available more information. The care algorithm provides an overview of all steps involved in a comprehensive clinical assessment.

It always includes four steps:

Step 1: The physician provides a detailed description of the history and physical examination, including preliminary tests already available, reports of comorbidities, and previous recommendations given to the patient.

Step 2: Comprises the reporting of invasive diagnostic methods such as fine needle aspiration, biopsy and/or surgery, review of the pathologic exam, immunohistochemistry and molecular markers.

Step 3: The physician integrates clinical, morphological and molecular data, which leads to a diagnosis. At this point, an assessment of the extent of the disease (staging) is also performed. Here, comorbidities are carefully considered. A performance scale is applied, evaluating the patient's clinical condition to face the proposed treatment. The prognosis is discussed with the patient and family. 

Step 4: Consists of the medical recommendation, indicating a therapeutic plan and describing its level of evidence. A rehabilitation plan is designed for the full recovery of the patient. When hereditary risk is identified, the doctor recommends genetic counseling.

During the entire process, the patient is informed about the findings obtained at each stage. The doctor takes a cognitive approach using simple language. Multimedia resources are used to improve patient understanding.

The method naturally leads to patient-centered shared decision making.
